For your convenience, we accept Visa, MasterCard, Discover and Care Credit.  We also accept cash or a personal check drawn from a local bank. There is a $30.00 fee for a returned check.

We are participating members of several insurance companies. However, patients with these insurances will be required to meet the deductible, co-payments and fees over the yearly maximum as well as non-covered services if applicable.

We will ask you to sign a financial responsibility statement reflecting acknowledgment and understanding of this policy at your initial office visit.
